The Clinical Documentation Integrity Specialist (CDIS) is responsible for evaluating the quality, accuracy, and completeness of provider documentation in the medical record. The CDIS uses advanced clinical knowledge and a strong understanding of ICD-10 coding guidelines and documentation standards to perform comprehensive, concurrent reviews. 

 

Essential Responsibilities:

  1. Conduct concurrent reviews of clinical documentation to assess completeness, clarity, and consistency for coding accuracy, quality outcomes, and compliance.
  2. Complete initial chart reviews within 24–48 hours of admission; perform continued stay reviews every three (3) acute care days for targeted patient populations.
  3. Analyze the full medical record to identify discrepancies or documentation gaps, including review of labs, imaging, nursing/therapy/dietary notes, consults, physician orders, and interventions.
  4. Issue compliant physician queries per ACDIS Guidelines when documentation is ambiguous, inconsistent, or lacks clinical support.
  5. Assign working MS-DRGs using ICD-10-CM/PCS guidelines, including correct identification of principal diagnoses, secondary conditions, and procedures.
  6. Participate in reviews supporting quality metrics such as Hospital-Acquired Conditions (HACs), Elixhauser Comorbidities (ELIX), and expected mortality indicators.
  7. Collaborate with providers, coding professionals, and interdisciplinary teams to ensure alignment of documentation and coding.
  8. Utilize the electronic health record (EHR) and CDI software for concurrent reviews, query generation, and workflow documentation.
  9. Meet or exceed performance targets related to review volume, query accuracy, and documentation quality.
  10. Stay informed of industry best practices, regulatory changes, and updates to coding standards.
  11. While this position can be performed remotely, flexibility in hours may be required for provider collaboration and documentation discussions.

Minimum Qualifications

Education:

  • Graduate of an LPN certificate/diploma, or
  • RN (Associate’s or Bachelor’s Degree), or
  • Health Information Management (Associate’s or Bachelor’s Degree)

 

OR

 

Licensure/Certifications:

  • Active Registered Nurse (RN) or Licensed Practical Nurse (LPN) license (Georgia)
  • OR RHIA/RHIT credential with CCS certification
  • CDIP or CCDS preferred (required within two years of hire)

 

WITH

Experience:
