Key Responsibilities:

  • Remotely provide a high level of care to patients with diabetes, hypertension, hyperlipidemia, and obesity remotely via phone and video chats. 
  • Develop personalized nutrition and lifestyle plans customized to each patient's medical conditions and goals. 
  • Manage patient medication adjustments in relation to their medical conditions in partnership with their pharmacist.
