• Oversees daily operations of interdisciplinary teams across Day Health Center and home care settings

• Directs care planning, staff supervision, and resource allocation to ensure efficient service delivery

• Serves as the Director of Patient Services (DPS) for the LHCSA, ensuring compliance with DOH regulations and policies

• Leads staff training and professional development initiatives to maintain high standards of care

• Monitors quality metrics and cost-efficiency to drive improved participant outcomes and program effectiveness
• Monday - Friday 8:30 am - 5:00 pm

  Required Qualifications for the Clinical Service Director:

• Clear and active RN license in New York State
• 3+ years of recent Certified Home Health Agency (CHHA) experience
• 3+ years of managing a team of Registered Nurses
• Recent experience working with the elderly

• This role involves conducting CT scans under the guidance of a Radiologist to aid in diagnosing medical conditions in patients of all ages.
