• Initiates and applies appropriate preventative, therapeutic and rehabilitative nursing procedures and techniques.
• Administers medications and treatments as prescribed by the PCP. Ability to independently perform skilled nursing procedures and techniques based on the changing nursing needs that the member requires.
• Teaches the patient and family/caregivers self-techniques whenever appropriate and provides instruction regarding medication, diet, safety and treatment modalities in accordance with the plan of care.

Shift: Monday – Friday 8:30 AM - 5:00 PM Required Qualifications for Field Registered Nurse:

• Must be willing to travel within field and go to patients’ homes and have home health or field experience
• A current and clear New York State License to practice as a Registered Nurse.

Field Case Manager – Registered Nurse

HealthCare Support is seeking a Field Case Manager – Registered Nurse for a mission-driven organization in Minneapolis that’s dedicated to empowering individuals with complex health needs to live safely and independently in their communities.  Pay Details: $40/hr. – $43/hr. Shift Details: Monday – Friday, 8:00 AM – 4:00 PM I No weekends or on-call! Daily Responsibilities for Field Case Manager – Registered Nurse 

• Conduct home visits to assess client needs, develop care plans

• Serve as a resource for clients, families, and staff on care coordination, program use, and community services to promote client health and safety

• This role involves no direct patient care, great for someone who is trying to step away from a traditional bedside role! 

 

Required Qualifications for Field Case Manager – Registered Nurse :

• Current unencumbered license as a Registered Nurse in the state of Minnesota
• 1+ year of experience as a Registered Nurse
• Prior experience in home health or field-based role
• Valid driver’s license, clean driving record, and reliable transportation to use for work driving purposes
