  • The Care Coordinator supports patients and prescribers by answering medication-related questions, coordinating prescription refills, updating billing information, assisting with reimbursement and pharmacy claims, obtaining authorizations and overrides when necessary, and discussing financial responsibilities with patients. They also address patient concerns and provide Spanish translation as needed.
  • Shift: HYBRID - Monday-Friday & No Weekends!
Required Qualifications for Bilingual Patient Care Coordinator:
  • High school diploma or graduate education degree (GED)
  • Bilingual – Fluent in English and Spanish, especially in medical terms.
  • Minimum of 1 year of pharmacy customer service experience

  Required Qualifications for Manager of Care Delivery – RN
  • Active NY Registered Nursing license
  • 5 years of clinical experience
  • Leadership experience
  • 3 years of home care experience 
Preferred Qualifications for Manager of Care Delivery – RN
  • Bachelor of Science in Nursing
  • Completed NYSED-approved infection control coursework
  • Managed long-term care insurance experience beneficial
  • Case Management Certification recommended within 2 years of the employment
