Remote Clinical Care Specialist - Registered Dietitian

We are seeking compassionate and experienced dietitians committed to remotely care for individuals with cardiometabolic conditions. You will be a key member of a team dedicated to improving patient health by offering education, leadership, and support. In this remote position you will collaborate with physicians, patient care advocates, and pharmacists to achieve positive results for your patients.

Key Responsibilities:

  • Remotely provide a high level of care to patients with diabetes, hypertension, hyperlipidemia, and obesity remotely via phone and video chats. 
  • Develop personalized nutrition and lifestyle plans customized to each patient's medical conditions and goals. 
  • Manage patient medication adjustments in relation to their medical conditions in partnership with their pharmacist. 
  • Track patient progress through periodic check-ins, adjusting their plan as needed based on statistical outcomes and feedback.
  • Offer coaching, and consolations to patients in order to achieve sustainable changes in their diet, exercise routine, and the reduction of stress. 

Required Qualifications:

  • Minimum 5 years of experience in direct patient care.
  • Licensed Registered Dietitian Nutritionist.
  • Experience in medication management.
  • Strong communication skills with the ability to build trust and rapport with your patients and physicians.  

Preferred Qualifications:

  • Experience working with digital platforms focused on managing health conditions.
  • Multilingual, preferably in Spanish, to serve diverse populations.
  • Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM), or eligibility for certification.
