Field Nurse Practitioner – up to $150,000/year + bonus!
HealthCare Support is seeking a
Field Nurse Practitioner (NP) to join a rapidly growing healthcare provider serving patients in
Casa Grande, AZ.
This is a unique opportunity to deliver high-quality, patient-centered care directly in the home—focusing on Chronic Kidney Disease and primary care needs. If you’re passionate about building relationships, improving outcomes, and working with autonomy while still being part of a collaborative team, this role is for you.
Enjoy a dynamic mix of in-home visits and telehealth opportunities, all while making a real difference in underserved populations.
Schedule: Monday – Friday, 8:00 AM – 5:00 PM
Compensation Notes: $140,000 – $150,000 annually + 10% annual bonus
• Mileage reimbursement provided
Daily Responsibilities for the Field Nurse Practitioner:
• Conduct comprehensive in-home patient visits, including history and physical exams
• Develop and implement individualized treatment plans
• Order and interpret diagnostic tests and lab work
• Prescribe medications as appropriate
• Provide ongoing patient education and chronic disease management
• Complete accurate and timely documentation of services
• Collaborate with interdisciplinary care teams and participate in routine meetings
• Utilize telehealth to support patient care when appropriate

Required Qualifications for the Field Nurse Practitioner:
• Active Nurse Practitioner (NP) license in Arizona
• 2+ years of experience as a practicing NP
