RN Case Manager – $10,000 Sign On Bonus
We are seeking a compassionate and detail-oriented
RN Case Manager to join a leading acute care hospital in
Tallahassee, Florida. This is an excellent opportunity to support patients through comprehensive care coordination, discharge planning, and collaboration with interdisciplinary teams to ensure high-quality outcomes and appropriate transitions of care.
Shift Options: Days or Evenings
Compensation: $34.25 – $47.10/hour + $10,000 Sign on bonus

Required Qualifications for RN Case Manager:
- RN License
- 3+ years of RN experience in an acute care setting from one of the following units: Med/Surg, Telemetry, Neuro, ICU, IMCU, PCU, or ED
Daily Responsibilities for RN Case Manager:
- Coordinate patient care across the continuum, including assessment, discharge planning, utilization review, and collaboration with interdisciplinary teams to ensure appropriate level of care and timely transitions
- Advocate for patients by facilitating communication between providers, patients, families, and payers while maintaining accurate, compliant clinical documentation
