Compensation Notes:

• $32-$43.20/hour + shift differentials if applicable
• $10,000 Sign On Bonus! 

Daily Responsibilities for Renal Vascular RN:

• Collect comprehensive data pertinent to the patient’s health or situation
• Analyze the assessment data to determine the diagnosis or issues

Required Qualifications for Renal Vascular RN:

• Florida RN license
• 1+ year of RN experience in Rehab, Acute, or international experience required

RN Field Case Manager HealthCare Support is actively seeking a RN Field Case Manager  to fill an opening in Salem, Oregon. Daily Responsibilities for RN Field Case Manager

• Ensure accurate and complete documentation.
• Communicate and collaborate with the interdisciplinary care team
• Assist the provider/team with various care coordination activities
• Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
• Monday through Friday 8AM - 5PM

Required Qualifications for RN Field Case Manager

• Active and unrestricted RN license in Oregon
• Case management background preferred- experience working in a Long-Term Care facility as an RN preferred
• Associate degree or higher in Nursing (RN)

Preferred Qualifications for  RN Field Case Manager

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Proficient computer skills including the ability to document medical information with written and electronic medical records
