Nurse Practitioner HealthCare Support is actively seeking an Nurse Practitioner to fill an opening in Seattle, Washington. Daily Responsibilities for Nurse Practitioner:

• Conduct comprehensive assessments
• Effectively manage acute and chronic medical and behavioral conditions
• Ensure accurate and complete documentation, including ICD 10 conditions
• Monday to Friday 8am-5pm

Required Qualifications for Nurse Practitioner:

• Active and unrestricted license in Washingtonn (NP or PA)
• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center

Preferred Qualifications for Nurse Practitioner:

• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions

• Pay-Per-Visit Model | Annual Earnings Typically $87,000-$100,000
• Mileage Reimbursement

Schedule for Home Health RN:

• Monday-Friday 8am-5pm 
• On Call: Mon-Mon 1x a month

Daily Responsibilities for Home Health RN:

• Provide individualized, one-on-one patient care by developing and implementing care plans, applying nursing theories and techniques, assessing medications for effectiveness, and delivering services such as wound care, medication administration, and new admissions/start of care.

Required Qualifications for Home Health RN:

• Registered Nurse in Pennsylvania
• 1+ years of clinical experience as an RN
• Experience with IVs, Wound Care, and Disease Management (diabetes, CHF, liver disease, COPD)
• Must possess and maintain valid CPR certification while employed in a clinical role
• Must be a licensed driver with an automobile that is insured in accordance with state and/or organization requirements and is in good working order.
