Community Resource Coordinator - Field HealthCare Support is actively seeking a Community Resource Coordinator - Field to fill an opening with a Fortune top 25 Managed Care Company in Gwinnett County, GA. May travel up to 200 miles per day. Mileage is reimbursed. Daily Responsibilities for Community Resource Coordinator - Field:

• Provides support to members to connect them to known community and care resources in a cost- effective manner
• Supports the coordination of community outreach resources available to members and promotes awareness of care/services
• Serves as support for members on community and care resource inquiries and opportunities available to members
• Supports all member related correspondence and educational materials to assist in the facilitation of a successful community connection
• Documents and maintains all community resources to ensure standards of practice and policies are in accordance with health plan requirements

Required Qualifications for Community Resource Coordinator - Field:

• Located in Gwinnett County
• Working Knowledge of Social Determinants of Health (SDOH) barriers
• Working Knowledge of geographic area(i.e. posted county/service area) and known Community Based Organizations
• Working knowledge of motivational interviewing and care plan development/goal setting
• General understanding of Microsoft office suite (i.e. work, outlook, excel)
• HS Diploma or GED
• Clear and active Driver’s license, reliable transportation, and car insurance

Preferred Qualifications for Community Resource Coordinator - Field:

• Bilingual highly preferred
• Community Health Worker Certification highly preferred
