Start & end times of shift: 8-5pm: Mon-Fri Pay: $45-50/hr (If you need to go up to $52, that is fine)  **Mileage is not reimbursed Primary Responsibilities:

• Ensure accurate and complete documentation.
• Communicate and collaborate with the interdisciplinary care team
• Assist the provider/team with various care coordination activities
• Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
• Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition
• Assist the provider / team with therapy coordination
• Participate in facility-based meetings
• Schedule and participate in family conferences, team meetings, and team case presentations
• Coordination of training, tracking, and compliance with quality measures
• Assist in creating and conduct in-services for selected audiences
• Assist Providers in assuring complete and accurate documentation and coding, medication reconciliation, nursing rounds, assessment, and patient documentation.
• May write verbal orders from Provider in the chart in compliance with the state RN practice laws, and in compliance with individual nursing facility practices
• Assist in managing members through an acute medical situation or an exacerbation of a chronic condition, including testing, diagnostics, interventions
• Assist in managing members during a short term sub-acute rehabilitation episode

Must have qualifications/experience:

• Active and unrestricted RN license in North Carolina
• Experience working in a Long-Term Care facility as an RN preferred
• Associate degree or higher in Nursing (RN)
• Certified in Basic Life Support
• Ability to complete physical requirements, including lifting a 30-pound bag and ability to perform physical assessment
• Access to reliable transportation

Preferred Qualifications:

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Proficient computer skills including the ability to document medical information with written and electronic medical records
• Effective Communication skills and time management
• Ability to work with diverse care teams in a variety of settings
• Access to reliable transportation
• Ability to travel in between facilities

Part-Time Healthcare Patient Engagement Specialist (Remote Florida or EST Time Zone states) HealthCare Support is actively seeking multiple Healthcare Call Center Patient Engagement Specialists to fill several large start classes with our prestigious healthcare client Millenium Physician Group (MPG).  These roles will work remotely in Florida or other EST Time Zone states.    Daily Responsibilities for Patient Engagement Specialist:

• Manage incoming phone-based and electronic patient communications, including scheduling requests, appointment changes, general inquiries, and care access needs.
• Apply provider template rules, visit type requirements, payer or referral considerations, and site-specific standards to schedule accurately.
• Verify and update patient demographics, insurance information, contact preferences, and required appointment documentation.
• Route clinical messages, prescription requests, symptom concerns, and non-routine patient questions to the appropriate care team or licensed clinical staff.
• Training will be 3-4 weeks.
• 11/16 and 11/30 Start Dates
• 5 Spots - Mon/Tues 8 am - 5pm and Wed/Thurs/Fri 8am to 12pm
• 5 Spots - Mon/Tues 8 am - 5pm and Wed/Thurs/Fri 1pm to 5pm
• Length: Contract until end of March timeframe.  There is a potential for some of these roles to convert perm for the high performers.

Required Qualifications for Patient Engagement Specialist:

• High school diploma or GED required.
• 2+ years of experience as a healthcare call center/member service/scheduler. 
• Working knowledge of appointment scheduling, insurance verification, patient communication, care team message routing, and electronic medical record documentation.

Preferred Qualifications for Patient Engagement Specialist:

• Bilingual English/Spanish.
• Experience using scheduling systems, electronic medical records, call center platforms, and patient communication tools.
• Athena EMR.
