Community Health Worker   HealthCare Support is actively seeking multiple Community Health Worker to fill a large start class October 12th with our prestigious healthcare client in Ohio.    Daily Responsibilities for Community Health Worker

• Engage with the member in a variety of settings to establish an effective, professional relationship. Settings for engagement include but are not limited to; hospital, provider office, community agency, member’s home, telephonic or electronic communication
• Accompany members to appointments and other social service encounters when necessary
• Coordinate logistics to support members’ care plan goals and interventions – reminders, transportation, and childcare arrangements
• Monday to Friday 8AM - 5PM

Required Qualifications for Community Health Worker

• High School Diploma or General Education Diploma (GED), is required
• Minimum of two (2) years of experience in either volunteer or paid position working in community settings with at risk populations providing coordination of services is preferred
• Community Health Worker Certification, or equivalent approved training program, is preferred

Dermatology Advanced Practice Provider (NP/PA) – Leland, NC  HealthCare Support is seeking Nurse Practitioners (NP) or Physician Assistants (PA) for an immediate opportunity, with a mission-driven, community-focused healthcare organization in Leland, NC.  This dermatology program is focused on medical dermatology, offering providers the opportunity to build meaningful patient relationships while practicing at the top of their license in a supportive, well-resourced environment. Compensation:

• $140,000 - $160,000 annual salary (based on experience)

Schedule:

• Full-time, Monday-Friday (Days)

Daily Responsibilities:

• Provide medical dermatology care, evaluating and managing common skin conditions, developing treatment plans, prescribing medications, and ordering/interpreting diagnostic testing.
• Manage patient care independently or semi?independently, including conducting complete histories and physicals, educating patients on conditions and preventive care, and documenting all encounters accurately in the EMR.

Qualifications:

• Active Nurse Practitioner or Physician Assistant license in North Carolina
• Dedicated dermatology-focused experience required
