RN Field Assessor
HealthCare Support is actively seeking a to
fill
RN Field Assessor an opening in
Batesville / Mountain View, Arkansas.
Daily Responsibilities for RN Field Assessor:
- Conduct in-home or community-based RN assessments to evaluate the member’s health status, functional abilities, medications, and care needs, documenting findings accurately and on time.
- Develop and coordinate individualized care plans in collaboration with the member, caregivers, providers, and health plan team, including referrals to appropriate services and resources.
- Monitor for changes in condition and safety risks, provide patient education, and communicate updates/escalations to the appropriate clinical leadership while maintaining compliance with all regulatory and confidentiality standards.
- Monday to Friday 7AM / 7:30AM - 4:30PM
Required Qualifications for RN Field Assessor:
- Licensed RN in Arkansas
- 1 year of RN experience
- High level of computer literacy and Competent Typing
- Competent Driving Skills
Preferred Qualifications for RN Field Assessor:
- PTCB certification
- MTM or Medication Adherence experience preferred

Pay Details: $40 - $42/ Hour
