Physical Therapist 

HealthCare Support is seeking a highly motivated and compassionate Home Health PT Physical Therapist to support a fulltime opening in Sarasota, FL. You will be part of a dynamic Clinical Team who ensures that our patients return and maintain their optimum level of physical functioning

Daily Responsibilities for the Home Health Physical Therapist:

• Perform initial and ongoing assessments in home, including OASIS assessments at appropriate time points, to determine level of functioning and patient’s ability to perform activities of daily living
• Develop and revise plans of care, in consultation with the physician and other care team members, which will enable patient to achieve realistic goals of mobility

Schedule:

• Monday – Friday | 8:00am – 5:00pm 

Compensation:

• Pay Per Visit (Annual Salary Range of $105-115K)

Required Qualifications:

• DPT from CAPTE accredited program
• Licensure as a physical therapist in the state of Florida
• 1+ years of experience in home health care
• BLS from the American Red Cross or American Heart Association
