Senior UAS Assessor - Steuben County HealthCare Support is actively seeking a Senior UAS Assessor to support members in Steuben County, NY. This organization is known for its commitment to high-quality, community-based care management and its focus on accurate, timely assessments to support long-term services and supports. In this role, the assessor will conduct comprehensive UAS-NY assessments, collaborate with care teams, and help ensure members receive appropriate, high-quality services—offering a meaningful position with direct impact on patient outcomes. Schedule:
  • Monday – Friday, 8:30 AM – 5:00 PM
Compensation:
  • $90,000 – $95,000 annually (DOE)
  • Mileage & toll reimbursement
Daily Responsibilities for a Senior UAS Assessor:
  • Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes.
  • Develop and manage personalized long-term care plans while educating members and families on available services and benefits.

Required Qualifications for Senior UAS Assessor:
  • Active and clear NY State RN license
  • 2+ years of recent experience as a RN 
  • Must have Home Health or UAS experience
