RN Clinical Case Manager
HealthCare Support is actively seeking a
RN Clinical Case Manager to fill an opening in
Wauwatosa, Wisconsin.
Daily Responsibilities for RN Clinical Case Manager
- Ensure accurate and complete documentation.
Communicate and collaborate with the interdisciplinary care team
- Assist the provider/team with various care coordination activities
- Assist the provider/team with benefit determination associated with Medicare part A, part B, and part D benefits
- Responsible for collaborating with the primary advanced practice clinician, (APC) and facility to identify and respond to patient change in condition

M-F 8:30-5pm
Required Qualifications for RN Clinical Case Manager
- Active RN license in WI
- Case management/ strong assessment skills
- Associate degree or higher in Nursing (RN)
- Certified in Basic Life Support
Preferred Qualifications for RN Clinical Case Manager
- 1+ year of clinical experience in practice (long-term care setting preferred)
- Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
- Understanding of Geriatrics and Chronic Illness
- Proficient computer skills including the ability to document medical information with written and electronic medical records

Pay Details: $45 - $50/Hour
