Daily Responsibilities for Nurse Practitioner/Physician Assistant - Float: Principal Duties and Responsibilities:

• Conduct comprehensive physical assessments and health screenings for patients across the lifespan.
• Diagnose and treat common acute and chronic medical conditions within the scope of practice.
• Prescribe medications and provide patient education on proper use, side effects, and lifestyle modifications.
• Perform minor procedures such as wound care, immunizations, and point-of-care testing.
• Document patient care accurately in the electronic health record (EHR) system.
• Promote preventive care through health counseling and routine screenings (e.g., vaccines, wellness exams).
• Collaborate with other healthcare professionals and refer patients to appropriate specialists as needed.

Required Qualifications for Nurse Practitioner/Physician Assistant - Float:

• Clear and active NY NP or PA license
• 1-2 years of experience 

Preferred Qualifications for Nurse Practitioner/Physician Assistant - Float:

• At least 1-2+ years of experience
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
• Understanding of Advanced Illness and end of life discussions
• Proficient computer skills including the ability to document medical information with written and electronic medical records

• Active and unrestricted license in New York (NP or PA)
• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program
• NP: Board certified through the American Academy of Nurse Practitioners or the American Nurses Credentialing Center, with certification in one of the following:
• Family Nurse Practitioner Adult Nurse Practitioner Gerontology Nurse Practitioner, Adult-Gerontology or Acute Care Nurse Practitioner
• Current, active DEA licensure/prescriptive authority
• Ability to complete physical requirements of the job including lifting a 30-pound bag in and out of the car, navigating stairs and a variety of dwelling conditions, and sit, stand, and kneel to perform physical assessment
• Access to reliable transportation
• Ability to gain a collaborative practice agreement, if applicable in your state
