RN Telephonic Case Manager (Geriatric) HealthCare Support is actively seeking multiple RN Telephonic Case Managers (Geriatric) to fill remote openings throughout multiple metropolitan areas with our prestigious healthcare client.   Major metropolitan areas include Miami, Tampa, Atlanta, Columbus, Cincinnati, Indianapolis, St Louis, New York, Nashville, Raleigh Durham, Houston, and Denver.   Daily Responsibilities for RN Telephonic Case Manager (Geriatric):

• Responsible for collaborating with healthcare providers and/or consumer to drive personalized health management and improve health outcomes for optimal consumers.
• Performs care management activities within the scope of licensure for members with complex and chronic care needs.
• Ensures medically appropriate, high quality, cost effective care through assessing the medical necessity of inpatient admissions and extensions of stay, outpatient services, out of network services, and appropriateness of treatment setting and level of care.
• Monday - Friday 8AM to 5PM / Remote Timezone of residence

Required Qualifications for RN Telephonic Case Manager (Geriatric):

• Graduate from accredited nursing school 
• Compact RN license in state they live in
• 3 years of clinical experience
• 1-2 years of experience working with geriatric patients/members
• 2 years of TELEPHONIC case management experience required
