RN Case Manager HealthCare Support is actively seeking an RN Case Manager to fill an opening in Bangor, Maine. Daily Responsibilities for RN Case Manager:
  •  Conduct comprehensive admission and ongoing case management assessments, including medical necessity, level of care validation, and progression of care.
  • Drive lengthofstay optimization and access day performance through proactive discharge planning and daily interdisciplinary collaboration.
  •  Ensure CMScompliant reviews and form securement, including timely admission reviews, continued stay determinations, IMM and MOON administration.
  • Monday - Friday 8AM to 4:30PM
Required Qualifications for RN Case Manager:
  • RN license
  • Case Management experience
  • Patient facing/bedside presence
