Field Care Manager HealthCare Support is actively seeking a Field Care Manager to fill an opening in Orange, California. Daily Responsibilities for Field Care Manager:
  • Develops and implements a member’s specific care plan which includes prioritized SMART goals.
  • Care plan will be continuously reviewed, modified and updated to reflect the member’s needs, at minimum, annually or upon change in condition.
  • Schedules follow-ups to assess progress towards goals and identifies barriers to meeting goals.
  • M-F, 8-5 PST (need to be flexible to work evenings and/or weekends when needed)
Required Qualifications for Field Care Manager:
  • Bachelor's degree in social work, psychology, gerontology, public health, or social sciences
  • 5+ years of clinical or managed care experience required
  • Valid driver's license, reliable transportation, active auto insurance, and ability to pass DMV test
  • 2+ years of experience working with either/both seniors and people with disabilities
