Community Health Worker / Care Coordinator HealthCare Support is actively seeking a Community Health Worker / Care Coordinator to fill an opening. Daily Responsibilities for Community Health Worker / Care Coordinator:

• This team supports member outreach, care coordination, and preventive health efforts for WellCare of Kentucky.
• These openings are driven by business need and increased workload to support outreach goals and member engagement.
• The team is collaborative, fast-paced, and focused on helping members access needed care and services. Key projects include VBP initiatives of outreaching members with Wellchild, Postpartum, A1C and Breast Cancer care gaps.
•  Monday to Friday 9AM-5:30PM in EST / Remote

Required Qualifications for Community Health Worker / Care Coordinator:

• HS Diploma or GED
• Community Health Worker Cert is strongly preferred
• 3+ years of healthcare customer service experience from a health plan, community health, or provider setting
• 1+ years of Experience Coordinating Care for patients, especially looking for people who have coordinated care for geriatric patients
