Community Health Worker / Care Coordinator
HealthCare Support is actively seeking a Community Health Worker / Care Coordinator to fill an opening.
Daily Responsibilities for Community Health Worker / Care Coordinator:
This team supports member outreach, care coordination, and preventive health efforts for WellCare of Kentucky.
These openings are driven by business need and increased workload to support outreach goals and member engagement.
The team is collaborative, fast-paced, and focused on helping members access needed care and services. Key projects include VBP initiatives of outreaching members with Wellchild, Postpartum, A1C and Breast Cancer care gaps.
Monday to Friday 9AM-5:30PM in EST / Remote
Required Qualifications for Community Health Worker / Care Coordinator:
HS Diploma or GED
Community Health Worker Cert is strongly preferred
3+ years of healthcare customer service experience from a health plan, community health, or provider setting
1+ years of Experience Coordinating Care for patients, especially looking for people who have coordinated care for geriatric patients
