Home Health LPN / Licensed Practical Nurse – Loveland, CO

Compensation & Perks

Estimated Annual Earnings: $82,000 – $95,000 (pay-per-visit model)

Mileage Reimbursement: $0.60/mile

Schedule: Monday – Friday, 8:00 AM – 5:00 PM
- No on-call responsibilities

What You’ll Do
- Provide skilled nursing care to patients in their homes
- Manage a daily caseload of 7–8 visits (30–45 minutes each)
- Perform:
- Wound care & wound vac management
- IV therapy & medication administration
- Chronic disease management (CHF, COPD, diabetes, etc.)
- Educate patients and caregivers on treatment plans
- Complete documentation using a provided tablet (flexible work locations)
- Participate in weekly case conferences (currently virtual)
Qualifications
- Active Colorado LPN license
- Graduate of an accredited practical nursing program

At least 1 year of acute care experience (Home Health, Med-Surg, ED, SNF, etc.)
- Strong clinical skills in IV therapy, wound care, and disease management
- Valid driver’s license, reliable vehicle, and ability to travel up to a 50-mile radius
- Current CPR certification
