HealthCare Support is actively seeking a
Field Nurse Practitioner / Physician Assistant to fill a permanent opening with a well respected healthcare provider in
Maricopa County in
Arizona.
Pay Details:

$120,000 - $143,000/year
-
Mileage reimbursement
-
Up to 7.5% annual bonus
Schedule:
-
Monday - Friday, 8:00am - 5:00pm
-
On-Call: Telephonic Only, once every 6-8 weeks
Required Qualifications for a Field Nurse Practitioner / Physician Assistant:
-
Active NP License in AZ
-
Certified as a Family Nurse Practitioner or Adult Gerontology Nurse Practitioner or Physician Assistant
-
Minimum 1 year of experience in family practice, general practice, or outpatient care
-
Valid and Active Drivers License
Daily Responsibilities for Field Nurse Practitioner / Physician Assistant:

Conduct comprehensive in-home and community-based visits (minimum 25/week), including assessments, diagnoses, and treatment planning
-
Manage chronic conditions, prescribe/adjust medications, and provide patient education to support long-term health
-
Collaborate with healthcare teams to deliver coordinated, high-quality care and serve as the primary provider of record
