Field Nurse Practitioner / Physician Assistant HealthCare Support is actively seeking a Field Nurse Practitioner / Physician Assistant to fill a permanent opening with a well respected healthcare provider in Maricopa County in Arizona.  Pay Details:
  • $120,000 - $143,000/year
  • Mileage reimbursement
  • Up to 7.5% annual bonus
Schedule:
  • Monday - Friday, 8:00am - 5:00pm
  • On-Call: Telephonic Only, once every 6-8 weeks
Required Qualifications for a Field Nurse Practitioner / Physician Assistant:
  • Active NP License in AZ 
  • Certified as a Family Nurse Practitioner or Adult Gerontology Nurse Practitioner or Physician Assistant
  • Minimum 1 year of experience in family practice, general practice, or outpatient care
  • Valid and Active Drivers License 
Daily Responsibilities for Field Nurse Practitioner / Physician Assistant: 
  • Conduct comprehensive in-home and community-based visits (minimum 25/week), including assessments, diagnoses, and treatment planning
  • Manage chronic conditions, prescribe/adjust medications, and provide patient education to support long-term health
  • Collaborate with healthcare teams to deliver coordinated, high-quality care and serve as the primary provider of record
