Nurse Practitioner   HealthCare Support is seeking a Nurse Practitioner to fill an opening with a prestigious Healthcare company in Brooklyn, NY.   Daily Responsibilities for the Nurse Practitioner

• Ensure proper oversight and coordination of participant care with the Interdisciplinary Team
• Conduct comprehensive History and Physical on new/current participants
• Provide evaluation and treatment of participants during acute illness; manage participants’ chronic illness in collaboration with the interdisciplinary team
• Provide preventive health maintenance for participants
• Conduct initial and semi-annual assessments in various care settings; perform additional physical assessments on participants and implement appropriate interventions as needed.
• Will have a full caseload
• Shift: Monday - Friday 8:30 am - 5:30 pm

  Required Qualifications for the Nurse Practitioner

• Active and unrestricted NP License in the state of NY
• DEA Certification required
• AANP or ANCC Board Certification required.
• 2+ years in clinical nursing practice in-home care, care management, or related field required.
• 1+ year of experience working with a frail or elderly population

  Pay Details:

• $155k - $165k
