RN Case Manager - Transition of Care HealthCare Support is actively seeking a RN Case Manager - Transition of Care to fill an opening  in Williamsville, New York.  Daily Responsibilities for RN Case Manager - Transition of Care:

• The Transition of Care Case Manager will be responsible for applying the essential activities of case management (assessment, planning, implementation, coordination, monitoring, and evaluation) through the outreach and support of members experiencing a transition of care. They will also be responsible for employer sponsored biometric lifestyle change coaching.
• Monday to Friday 8AM - 4:30PM

Required Qualifications for RN Case Manager - Transition of Care:

• Ability to use multiple computer systems, phone, and technology platforms – please list what systems they have experience using including which electronic medical records systems they’ve used for patient documentation
• Clear and active NY RN license
• 3+ years of experience in ICU, CCU, PCU, NICU, oncology, transplant, med/surg or ER
