What You’ll Do As a , you will be responsible for: Verify prescriptions and oversee patient therapy for accuracy and safety Build provider relationships and coordinate patient care Conduct clinical assessments and deliver medication counseling to patients and caregivers Monitor patient therapy responses, document progress, and address adherence issues Collaborate with prescribers, nurses, care coordinators, and payers to optimize treatment plans Support individualized care plan development meeting URAC and ACHC standards Identify medication-related issues and participate in quality improvement initiatives Serve as clinical shift leader, guiding clinical and non-clinical staff Ensure compliance with federal, state, and accreditation requirements What You’ll Bring The ideal candidate for this role will have: Active Arizona State Pharmacist license in good standing Doctor of Pharmacy (Pharm.D.) degree Minimum 1 year of pharmacist experience or equivalent training Strong knowledge of state and federal pharmacy regulations Excellent communication skills with patients, providers, and team members Solid prescription verification background (specialty experience preferred) Strong documentation and collaboration skills Preferred Qualifications: Experience in specialty pharmacy or clinical care settings, especially with case management and polypharmacy At least 3 years of experience as a pharmacist, with a focus on managing pharmacy operations Minimum of 1 year working in an accredited specialty pharmacy setting Certification as a Specialty Pharmacist (CSP) is a plus Familiarity with URAC and ACHC specialty accreditation standards Outstanding problem-solving skills and the ability to work well with interdisciplinary teams Why Join Us in Gilbert?
