We are seeking a Staff Pharmacist for a home infusion pharmacy in North Charleston, South Carolina. This is a rapidly growing compounding pharmacy that specializes in acute home-infusion IV therapies, intravenous immunoglobulin (IVIG), and subcutaneous immunoglobulin (SCIG). The Pharmacist will dispense medications and medical equipment, assess therapy appropriateness, document patient progress, and provide medication education in accordance with company policies and regulatory requirements.
As a Pharmacist, you will be responsible for:
Provides safe and appropriate pharmacy services to patients in the alternate care setting in accordance with policies and procedures,
Compile patient information and develop individualized pharmacy care plans with therapy goals
Assesses patient's suitability for home care and appropriateness of prescribed therapy.
Review laboratory results and perform pharmacokinetic dosing.
Counsel patients and caregivers on prescribed therapy, medication use, and infusion methods
The ideal candidate for this role will have:
Bachelor of Science in Pharmacy required; Doctor of Pharmacy or Master's of Science in Pharmacy preferred
Current state pharmacy license required
Minimum 2 years recent acute-care experience OR 1 year recent home infusion therapy experience
Strong knowledge and experience with intravenous admixture services and IV therapy

As a Pharmacist, you will be responsible for:
Verify prescriptions for accuracy, safety, and formulary alternatives
Provide personalized counseling, MTM, and wellness services
Supervise and train the team of pharmacists and pharmacy technicians

The ideal candidate for this role will have:
Pharm.D. with active Ohio pharmacist license required
