IDR is seeking a Case Manager to join one of our top clients for an opportunity in the Sunnyvale, TX area. 

Overview:

The Case Manager will support the Case Management team by coordinating patient care, managing discharge planning, and collaborating across clinical teams. This is a weekday-only position, ideal for someone with strong interpersonal skills, attention to detail, and a passion for improving patient outcomes in an acute care setting.

Must-Have Qualifications:

• 1-3 years of Case Management Experience  
• Certifications: BLS
• Active TX RN License or LMSW / LCSW - TX License 

Preferred Qualifications:

• Cerner Experience
