IDR is seeking a Case Management Assistant to join one of our top healthcare clients Remotely in PST or MST. If you are looking for an opportunity to join a large organization and work within an ever-growing team-oriented culture, please apply today!   Position Overview for the Case Management Assistant:  
  • Transition of Care Coordination: Facilitates and implements the transition of care and discharge plans for ambulatory patients, collaborating with case managers, social workers, patients, and families to ensure timely and efficient discharges.

  • Communication and Documentation: Maintains accurate, current, and legible documentation in the electronic medical record, capturing key discussions and updates on the status of referrals and discharge plans. Provides necessary clerical support, including copying, faxing, scanning, and data entry.

  • Collaboration and Teamwork: Works closely with the care coordination team, participating in daily huddles and departmental meetings. Proactively identifies and resolves barriers to the transition of care plans, ensuring effective communication and problem-solving.

  • Customer Service and Compliance: Builds positive relationships with customers and partners, demonstrating respect and effective communication skills. Complies with confidentiality policies, HIPAA regulations, and department standards when handling patient information.

Required Skills for the Case Management Assistant:  
  • Education and Experience: High School Diploma or equivalent education/experience. A minimum of 1-year recent relevant experience in a similar role. Experience with Epic Electronic Health Records is required. 

  • Communication Skills: Strong oral and written communication skills, capable of interacting effectively with diverse groups, including patients, families, and healthcare providers.

  • Organizational and Time Management Skills: Ability to work effectively in a fast-paced environment with shifting priorities and competing demands. Demonstrates strong time management and organizational skills, with the ability to prioritize tasks and work independently.

  • Technical Proficiency: Intermediate PC skills and word processing skills. Experience with electronic medical records and other healthcare-related software systems, ensuring accurate data entry and documentation.
