HealthCare Support is actively seeking a full-time
Senior UAS Assessor to support members in
Albany, NY. This organization is known for its commitment to
high-quality,
community-based care management and its focus on accurate, timely assessments to support long-term services and supports. In this role, the assessor will conduct comprehensive UAS-NY assessments, collaborate with care teams, and help ensure members receive appropriate,
high-quality services—offering a meaningful position with
direct impact on patient outcomes.
Schedule:
- Monday – Friday, 8:30 AM – 5:00 PM
- Work-life balance: No weekends, no holidays, no night shifts
Compensation Notes:
-
$90,000 – $97,000 / Annually
- Annual Bonus Incentive
- Mileage & toll reimbursement
Daily Responsibilities for Senior UAS Assessor:
- Assess and coordinate medical and support services across the continuum of care for complex, high-acuity populations to promote quality, cost-effective outcomes.
- Develop and manage personalized long-term care plans while educating members and families on available services and benefits.
Required Qualifications for Senior UAS Assessor:
- Active and clear NY State RN license
- 2+ years of recent experience as a RN
- Must have Home Health or UAS experience
