Nurse Practitioner / Physician Assistant HealthCare Support is actively seeking a Nurse Practitioner / Physician Assistant to fill an opening in Smithtown, New York. Daily Responsibilities for Nurse Practitioner / Physician Assistant:

• Conduct comprehensive assessments Effectively manage acute and chronic medical and behavioral conditions Ensure accurate and complete documentation, including ICD 10 conditions
• Monday to Friday 8AM - 5PM EST

Required Qualifications for Nurse Practitioner / Physician Assistant:

• Active and unrestricted license in New York (NP or PA)
• Needs to have minimum 1 year working experience as an NP or PA (they will not accept new grads with no experience)
• NP: Graduate of an accredited Master of Science Nursing or Doctor of Nursing Practice program

Preferred Qualifications for Nurse Practitioner / Physician Assistant:

• 1+ year of clinical experience in practice (long-term care setting preferred)
• Experience in meeting the medical needs of patients with complex behavioral, social and/or functional needs
• Understanding of Geriatrics and Chronic Illness
