Specific Job Duties- 

1. Recognize and overcome individual patient obstacles hindering adherence to medication 
2. Anticipate, identify, and resolve issues related to medications
3. Work closely with healthcare providers, as well as clinical and administrative personnel, to refine the process of medication management 
4. Adapt and suggest alternative medication options
5. Support healthcare providers, care coordinators, and other team members in overseeing high-risk, high-cost patients to ensure superior patient care.

Environment- fully remote 

Schedule- M-F, 9a-5p EST  hours 

Pay Rate: Negoiatable
