Job Title: Licensed Practical Nurse (LPN)

Location: Onsite

Contract Type: Contract to Hire Pay: $35/hr + $0.41 Mileage reimbursement Shift Requirements
  • Monday–Friday, 8:00 AM – 5:00 PM

  • Weekends may be required due to weather-related events or make-up appointments

  • No on-call requirement

Licensure & Certification Requirements
  • Active Licensed Practical Nurse (LPN) license in Alabama

Mileage Reimbursement
  • $0.41 per mile

Preferred Qualifications
  • Wound care experience, including wound vacs

  • Knowledge of heart failure and diabetes management

  • Ability to provide patient education on catheters and other disease processes

  • Injection experience

  • Experience with Homecare Homebase (HCHB) preferred

Job Summary

The Licensed Practical Nurse (LPN) provides direct patient care in accordance with individualized plans of care, physician orders, and organizational policies. The LPN promotes patient safety, dignity, and independence while delivering high-quality nursing services in a home health or clinical environment.

Key Responsibilities
  • Provide patient care in accordance with established plans of care and physician instructions

  • Monitor and document vital signs, reporting abnormal findings as appropriate

  • Administer medications, treatments, injections, feedings, oxygen therapy, and ostomy care as prescribed

  • Perform wound care, including management of wound vacs, as required

  • Educate patients and caregivers on disease processes, catheters, and treatment plans

  • Collaborate with interdisciplinary team members including Physicians, Physical Therapists, Occupational Therapists, and Speech Therapists

  • Maintain compliance with infection control policies and procedures

  • Identify, report, and document potential communicable diseases or infections

  • Accurately document all nursing care, patient responses, progress toward goals, and communications

  • Order and maintain adequate medical supplies, medications, and nutritional support

  • Monitor and assess patient incidents; complete incident reports accurately and in a timely manner

  • Ensure medical equipment is properly maintained and report malfunctioning equipment to the appropriate DME provider

  • Support patient safety, comfort, and independence at all times
