Job Title: Part-Time Clinical Specialty Pharmacist (Transplant Specialty)

Pay: $62+/hr Hours: Part-Time Start Date: ASAP

As a Clinical Specialty Pharmacist, you’ll play a key role in enhancing patient outcomes through comprehensive medication management and compassionate care. You’ll collaborate closely with healthcare professionals to ensure effective and safe medication therapy for transplant patients, helping to drive excellent healthcare results.

What You’ll Do:

As a Clinical Specialty Pharmacist, you will be responsible for:

• Verifying prescriptions and overseeing patient therapy for accuracy and safety
• Building provider relationships and coordinating patient care
• Conducting clinical assessments and delivering medication counseling to patients and caregivers
• Monitoring patient therapy responses, documenting progress, and addressing adherence issues
• Collaborating with prescribers, nurses, care coordinators, and payers to optimize treatment plans
• Supporting individualized care plan development meeting URAC and ACHC standards
• Identifying medication-related issues and participating in quality improvement initiatives
• Serving as a clinical shift leader, guiding clinical and non-clinical staff
• Ensuring compliance with federal, state, and accreditation requirements

What You’ll Bring:

The ideal candidate for this role will have:

• Active New York State Pharmacist license in good standing
• Doctor of Pharmacy (Pharm.D.) degree
• Strong knowledge of state and federal pharmacy regulations
• Excellent communication skills with patients, providers, and team members
• Strong documentation and collaboration skills

Preferred Qualifications:

• Experience in specialty pharmacy or clinical care settings, especially with case management and polypharmacy
• Familiarity with URAC and ACHC specialty accreditation standards
• Outstanding problem-solving skills and the ability to work well with interdisciplinary teams

Location & Schedule:

This position is on-site in Bronx, NY, with shifts from:

• 8:00 AM - 4:30 PM
• 9:00 AM - 5:30 PM
• 10:00 AM - 6:30 PM No weekend or holiday work is required as the pharmacy operates Monday through Friday.
