Job Title: Clinical Pharmacist (Remote)

Start Date: November 11th

Location: Remote (licensed in any state) Pay Rate: $60/hour

Key Responsibilities:

• Provide direct clinical care for patients with cardio-metabolic conditions.
• Patient Care: Provide direct clinical care to patients via telehealth, including medication management, adherence support, and education.
• Consultations: Conduct thorough consultations to assess patient needs and develop personalized care plans.
• Medication Management: Adjust medications as necessary under collaborative practice agreements, ensuring optimal therapeutic outcomes.
• Adjust medications for cardio-metabolic conditions under a collaborative drug treatment agreement (CDTA)

Requirements:

• Valid Pharmacist license in any state.
• Minimum of 5 years of experience in ambulatory care or clinical settings (VA’s, inpatient, primary care clinics)
• Experience working directly with patients and healthcare providers.
• Certified Diabetes Care and Education Specialist (CDCES) or Board Certified-Advanced Diabetes Management (BC-ADM), or eligible for certification.
• Flexibility to work varying shifts, including early mornings, evenings, or weekends.

Monday - Friday Shift Times:

• Shift 1: 6:30 am - 3:00 pm EST / 3:30 am - 12:00 pm PST

• Shift 2: 12:00 pm - 8:30 pm EST / 9:00 am - 5:30 pm PST

• Shift 3: 6:00 pm - 2:30 am EST / 3:00 pm - 11:30 pm PST

Sunday - Thursday and Tuesday - Saturday Shift Times:

• Shift 1: 6:30 am - 3:00 pm EST / 3:30 am - 12:00 pm PST

• Shift 2: 12:00 pm - 8:30 pm EST / 9:00 am - 5:30 pm PST
