Job Description: 

As a Pharmacy Outreach Specialist, you will play a vital role in ensuring patients receive their medications and adhere to their prescribed regimens. Your responsibilities will include reaching out to patients via telephone to identify and address barriers to medication adherence, as well as liaising with clinicians and pharmacies to facilitate the distribution of medications.

Key Responsibilities:

  • Conduct telephonic outreach to patients to assess and overcome medication adherence barriers.
  • Pursue new medications and refills by communicating with prescribers and patients via phone and text.
  • Ensure timely resolution of medication-related issues and provide one-call resolution for all inquiries.
  • Maintain a high level of professionalism in interactions with patients, providers, and colleagues.
  • Collaborate with team members to address any potential issues and maintain open lines of communication.
  • Conduct medication compliance calls and accurately record patient information in accordance with policies and procedures.
  • Educate patients on the importance of medication adherence and available resources.
  • Promote participation in Care Management programs and explain program features and benefits to patients.
  • Complete medication refill coordination with retail pharmacies, primary doctors, or other clinicians as required.
  • Handle prescription transfers and triage calls/voicemails to resolve outstanding prescription issues.
  • Maintain detailed record-keeping and generate reports related to outreach activities.
Must have requirements:
  • IL State CPHT License AND PTCB/NHA License is required
  • knowledge of diabetes oral meds, BP meds and Cholesterol meds - knowledge will be tested during an interview 
  • Basic knowledge of Excel is required
  • Great customer service skills and effective communication.
  • Looking for 3 years of retail or clinical pharmacy experience (or virtual pharmacies). Customer service and call center experience a plus, but not required.

Schedule- M-F 8a-5p CST, every Wednesday work from home, rest of the week in office. OT is not required and no weekends.

Pay Ranges- 21-25/hr, depending on experience 

Projected Start Date- 5/6 or 5/20 start dates , any pre planned time off in first 60 days (appointments/vacation) needs to be noted

Dress Attire- Business casual in office or scrubs

Schedule
