Job description

One of our major clients is looking for a LISW in the Cleveland area. This is a full time position with a competitive salary, benefits, and bonus! Schedule is Monday through Friday day shift hours overseeing elderly patient care.

SUMMARY:

Applies graduate-level clinical social work skills to the risk assessment of individuals, families, and groups and provides graduate-level clinical social work interventions as appropriate to client needs. Consults and collaborates with hospital and community professionals and programs, participates in program and organizational assessment, planning, and development according to license guidelines. Seeks professional social work supervision when appropriate for practice or training.

QUALIFICATIONS/REQUIREMENTS :

OH Licensed Social Worker with Master's Degree in Social Work from accredited Social Work academic program (Accredited by the Council on Social Work Education) Must have strong listening and communication skills and the ability to work well in an interdisciplinary setting. Computer skills are highly desirable. Must have two years social work experience, preferably in health care, and minimum one-year experience working with a frail/elderly population.

Job Type: Full-time

Salary: Up to $75,000.00 per year based on experience plus 10% bonus

Assess the appropriateness of medications on the basis of health conditions, indication, and therapeutic goals; evaluate the effectiveness, safety, and affordability of medication therapy in respect to patient specific factors such as age, weight, laboratory data and other procedures; identify medication-related problems.
